One thing I would like to see is a chance of getting clues when you rescue a survivor. After all, who knows if they lived through some of the horrors described by some of the clues you find? Survivors seem like they would be a good source of information.

Also, given the current rareness of puzzle rooms, I really don't want to see puzzles that you can permanently fail. That just doesn't sound like fun. The only puzzle idea that comes to mind right now, however, is a sort of ricochet puzzle. You'd need to aim a spell at the right angle so that it gets through the reflectors, and deflectors, to hit the switch on the other side.

Saddly I've yet to see any of these puzzle rooms, probably because i dont get to fully explore some of these areas because I'm forgetful of what I've explored or just waltzed in to. Though, by the sound of things, maybe throw in varied types? like a roller slide puzzle, or one of those line puzzles where the target object turns at the first horizontal line. i forget what those are called.

Actually, incorporating some of the above, you could have a very simple Lights Out puzzle - nine to twelve nodes, tops - but where a constant stream of enemies attempts to hinder your progress until the puzzle is complete and (perhaps) a time limit is enforced by the steady dripping of water/lava from ceiling to floor, filling the room.  :)

Lights out puzzle would be easy to implement. You can even reuse the same orbs. It could be as small as 3x3 or as large as you want. Could be randomly generated or could start with a certain pattern. Harder ones could have multiple colors, not just "on/off".

When I first saw a puzzle room I thought that's what the game was, until I hit one orb and none of the adjacent ones got flipped.
