Worst Situation you have landed yourself in (and whether you lived or not)

I'm interested in hearing stories about people who got themselves in a 'bad' way - here's one I'll share:

I've hung on quite tightly to my first character, but I needed to do some ocean crossing (without a wetsuit, or whatever the item is).  So I go the wrong way and to try and save time I teleport back to the starting area above the ocean.  Imagine my surprise when I'm on the bottom of the ocean.  I was a bat when this happened to and it felt like i was taking damage at an insane rate underwater.  Switching back to human and with about 88 health left I burn a small heal, and I then spot salvation!  Underground tunnels! It's a miracle I landed about 10 feet from the tunnel entrance.  I dive in and get the chunk invincibility to let my spells recover and reorganize.  More surprise again!  It's flooded under the ocean!  I sink only a small distance thankfully.

So I managed to get my health halfway to a safe margin and I'm back up on the bottom of the ocean and ready to start the risky climb to the top.  I think i have enough health items but that WAS the closest call I've had yet.  I'm still on the bottom of the ocean but it looks like I'll be able to get back up if I don't bump into too many nasties (i'm lvl10 monsters lvl 19).

Edit: I made it.  Just be careful about using teleport scrolls around oceans.  Lesson learned.

Ah, yes.  "Pull the lever."   It was magma, not just any sort of fire.

NEVER transmogrify to bat when under water. It seems like it would be so easy to fly to safety instead of platform jumping....

Not yet, but they're coming.  I've already done the carp graphics, but haven't coded them in yet. I find some references are fun to make, and there's a lot of AI War ones coming also, and already various ones to Chrono Trigger and other games.  I like little easter egg in jokes for folks who know the other things I like.  As I suspect anyone who's read the names of the achievements in AI War knows.

Well, I've already spoken of the one where I got killed by 2 shadow bats due to narrow space in another topic (I wasn't quite ready to battle in such conditions), but there was another misfortune that happened fighting a crippled dragon boss in a liutenant tower.

I had only saw the boss blip on the mini map, but couldn't figure which one was (though I had already fought 2 other dragons)... it was one of those vertical boss rooms, and when I land down there, *flame*, right in the middle of dragon breaths, and while I was able to escape most of them, their fire damage ultimately killed me.
In the same spot, I've almost died to my own ghost, unaware that the pink trail they leave was damaging also.
